The larger an organization gets, the less combat ability I'd expect from the average officer. In a feudal society, heredity or loyalty to the lord would be most important. If bigotry has a heavy foothold, one race might be elevated or suppressed. In a theocracy, a high priest or archbishop might lead. In a large nation with a precarious balance of power, expect political appointees. The best exception I can think of is a magocracy, in which the strongest spellcasters reign supreme because no one in the realm can oppose them; the Empire fits this category, though it only applies at the very top.
Contrast with a tribe of orcs where might makes right or, say, a mercenary company in which a Sergeant in motion outranks a Lieutenant who doesn't know what's going on (Maxim 2).
